4/9/2023 EnchantaVaughn Approved 1.Behavior: Jahsim greeted QMHP-C as QMHP-C arrived to the home. Jahsim appeared to be in a positive mood while discussing with QMHP-C how his day was going. He verbalized to QMHP-C that his day was going well and that he had a good day. Mother reports the negative behaviors contiued to be displayed by Jahsim such as him not getting along with peers and his siblings. 2. Behavior: During the second portion of the session client appeared to be agitated with mother when discussing his behaviors in the home. 1.Intervention: QMHP-C engaged Jahsim in self-controlling behaviors as well as how to control his behaviors. QMHP-C role-played with Jahsim on his peer interactions and triggers that causes him to act out inappropriately in the home setting. 2. Intervention: QMHP-C modeled effective ways to express feelings and thoughts accordingly and engaging in respectful behaviors. QMHP-C engaged Jahsim in controlling his frustrations and anger control. QMHP-C taught Jahsim different techniques to assist him with showing feelings rather than acting out the behaviors. 1.Response: Jahsim appeared to be receptive to QMHP-Cs’ interventions and services that were provided to him during the session. Jahsim struggles with self-controlling behaviors as he acts out impulsively without thinking of the consequences for his actions. 2. Response: Jahsim reported that he has been influenced by his peers as he would act out inappropriately with others. Through role-playing Jahsim was taught to identify his triggers and verbalize his thoughts.
